Surrogate Online Application


The Application Process
If you are interested in becoming a Surrogate for Surrogacy Options you should complete an online preliminary application. Once we have received and reviewed it we will contact you. You will be sent a package with additional forms and requests that you will need to complete and mail back to Dana.

After we receive your completed package, and you have been approved, we will introduce your profile to IPs. If they consider you a potential match, we will notify you and share the interested IP's profile with you. If you are interested as well, we will arrange a telephone conference with both parties. This gives everyone the opportunity to ask additional questions. The average length for a conference call is about one hour. After the call, we will be in contact with both the IPs and Surrogate to discuss additional issues or concerns. If you both agree to match then we will arrange a meeting for the IPs and the Surrogate and her spouse or partner.

If we do not have a potential match for you then we will advertise on your behalf in print and/or online. It can take time to find a perfect match, but it will happen. It is important to remember that any match is up to you.
